Concussion Baseline Testing Information
Erie - Prophetstown Athletic Coop in Association with Hammond Henry Hospital is once again offering free concussion baseline testing via the ImPact software.
Baseline concussion testing is mandatory for all incoming freshmen. Junior student athletes need to take the baseline test if they took it as freshman. Any sophomore going out for any extracurricular for the first time, also need to take the test. Remember, your baseline test is good for two years, therefore, if you participated last year, as an incoming sophomore or junior, you do not need to take the test. Please see below for the date and time slot assigned to your child via incoming grade level and last name. All ImPact Baseline Testing on June 6 will occur at the Erie High School Media Center. All ImPact Baseline Testing on July 29 will occur at the Prophetstown High School. Testing takes about 30 minutes.

11 AM – 11:30 AM Junior – last name starting with A – M
11:30 AM – 12 PM Junior – last name starting with N– Z
12 PM – 12:30 PM Senior – last name starting with A – M
12:30 PM – 1 PM Senior – last name starting with N – Z
1 PM – 2 PM Open, By appt. only (If necessary)
