Pre-K: Birth Certificate, Physical, Lead Level, and Vaccinations

Kindergarten: Birth Certificate, Physical, Lead Level, Vaccinations, Vision Exam- Due by October 1st of the current school year, Dental Exam- Due by May 1st of the Kindergarten school year

1st Grade: None

2nd Grade: Dental Exam- Due by May 1st of the 2nd grade school year

3rd Grade: None

4th Grade: None

5th Grade: None

6th Grade: Physical, Vaccinations-(TDaP, Meningococcal, 2nd dose Varicella if not previously done), Dental Exam - Due by May 1st of the 6th grade school year

7th Grade: 1st dose Meningococcal if not previously done

8th Grade: None

9th Grade: Physical, Up-to-date Vaccinations (2nd Varicella if not previously done), Dental Exam by May 1 of 9th Grade Year

10th Grade: None

11th Grade: None

12th Grade:  2 doses of Meningococcal vaccine at entry to 12th grade (Or one dose if first dose was received at 16 years of age or older)
