A reminder from the health office:  The Illinois Department of Public Health requires all students in Kindergarten, 2nd, 6th, and 9th grade to turn in proof of a dental exam by May 1st of the current school year.  The examination must be recorded on the approved Illinois State "Proof of School Dental Examination Form".    Forms can be found on the ECUSD #1 website. Examination must be done in the past 18 months so any exam on or after November 1, 2020 will work.  Forms completed by your dental office can be faxed to (309) 659-7254. Please notify the school nurses, Kelly at (309) 659-2239 X 1315,  or Shauna at (309) 659-2239 X 1216 if you have any questions or concerns.  There are Illinois Dental waiver forms available online as well if needed.  Thank you for your help in completing this requirement!
